Thread jumping out of take up lever groove when sewing?

Anyone know why the thread comes out of the thread take up lever groove? It’s been doing this for a while. I think this is the cause of my upper thread spool jumping resulting in a wad of thread on the bottom side of the fabric. It’s like it catches, pulling the upper thread which releases the “correct tension” then the extra thread caused by the tension not being there gets “sewn” onto the underside with the bobbin thread.
Any help appreciated.
Check out the “thread take-up lever”, this lever goes up and down where you see the thread popping out. This is a normal function and is common on all sewing machines.
